Statement of Educational Purpose (Institution)
This form is used to verify a student's identity. The form must be completed and submitted in person to the Financial Aid Office with a valid form of Identification. Can not be faxed, emailed, or mailed.
Statement of Educational Purpose (Notarized)
This form is used to verify a student's identity. The form must be notarized and submitted to the Financial Aid Office by postal mail with a copy of a valid form of Identification. Can not be faxed or emailed. This form can only be used by Distant Education students who live outside of a 90 mile radius of Citrus College, all others must submit the Statement of Educational Purpose listed above.

Appeal Application Form
This form is used to appeal your financial aid termination status as a result of not meeting Satisfactory Academic Progress (SAP).
Appeal Eligibility Criteria
Students dismissed from financial aid may appeal once per year if they meet the appeal eligibility criteria. Students with 120 units are not eligible to appeal.
This form is used to authorize Citrus College to use, or not use, financial aid funds to pay past, current or future institutional charges.
Consortium Agreement Form
This form is used by students who are enrolled in at least six units at Citrus College, and is enrolled in units at a secondary institution within the same semester.

Request for Release of Information (FERPA) Form
This form is used to give permission to the Financial Aid Office to speak to or release information to another person/persons you designate about your financial aid file. The Family Educational Rights and Privacy Act (FERPA) authorizes disclosure with written consent.
Special Circumstances Form
This form is used to request that your, your parent's, or spouse's, financial status be reviewed by the Financial Aid Office due to adverse circumstances. Students can only submit one special circumstance. If a student has a mitigating circumstances that might warrant a second special circumstance, it will require the review from the director of Financial Aid for consideration of approval on a case by case basis.
Statement of Fact
Use this form to clarify specific information requested by the Financial Aid Office.
Unusual Enrollment History Justification Form (UEH)
This form is used to explain any adverse circumstance that has caused a student to have an UEH over the last three aid years.
Work-In Progress Form
This form is used by students who have been requested by the Financial Aid Office to provide current grades for the classes they are enrolled in.
